Privacy Notice
Effective date: [EFFECTIVE DATE]
Draft for review. This notice describes what the website does today. Two things must be settled before it is published: the contact form has no backend connected yet, so the section describing where your details go is written for the intended setup rather than the current one, and your attorney should confirm the rights section against the laws that apply to your patients.
This notice explains what happens to information you give us through this website, renewveincenters.com, and what the site stores on your device.
It does not cover the medical records this practice holds about you. Those are covered by our Notice of Privacy Practices, which sets out your rights under HIPAA.
Who we are
- Renew Vein Center
- 515 Madison Avenue, New York, NY 10022
- Telephone: (833) 311-1274
What we collect
The only information this website asks you for is what you type into the contact form:
- Your name
- Your email address
- Your phone number, if you choose to give it
- Anything you write in the message box
Please do not use the contact form to send us detailed medical information. A website form is not a secure channel for health information. Tell us only enough to arrange a call or an appointment, and we will take the details properly from there.
Like almost every website, our hosting provider also keeps standard server logs, which include the IP address your request came from, the time of the request and the page you asked for. These are used to keep the site running and secure.
The contact form currently has no destination configured, so submissions are validated but not delivered. This section describes the intended behaviour and must be confirmed once a form provider is connected.
What we do with it
- To answer your enquiry and, if you want one, to arrange an appointment
- To keep a record of the enquiry so we can follow up
- To keep the website running, secure and working properly
We do not sell your information. We do not share it with advertisers. We do not use it to build an advertising profile of you.
Cookies and similar technologies
A cookie is a small file a website stores on your device. This site keeps its use of them to a minimum.
What the site stores:
- Your cookie choice. When you accept or decline on the cookie banner, we record that choice in your browser so we do not ask again on every page. This is stored on your device only and is never sent to us.
- Google Maps. The map on our contact section is supplied by Google, and Google sets its own cookies when the map loads. That is why the map does not load until you ask it to, or until you accept cookies.
- Google Fonts. Our pages load typefaces from Google. This does not set a cookie, but it does mean Google receives a request from your browser, including your IP address, when a page loads.
This website runs no analytics, no advertising pixels and no third party tracking scripts of any kind. If that changes, this notice must be updated before the new service goes live.
Changing your cookie choice
You can change your mind at any time using the "Cookie preferences" link at the bottom of any page. You can also clear cookies and site data through your browser settings, which will make the banner appear again.
Who else sees your information
- Our website hosting provider, which stores the site and its server logs
- The form provider that delivers contact form submissions to us, once one is connected
- Anyone we are required to share information with by law
These providers act on our instructions and are not permitted to use your information for their own purposes.
How long we keep it
We keep contact form enquiries for as long as we need them to deal with your enquiry and to keep a record of it, and then we delete them. Server logs are kept for a short period for security and troubleshooting.
Medical records are kept separately, for the periods required by New York State law and by our professional obligations.
[PLACEHOLDER: state the actual retention period for enquiries, for example twenty four months, once the practice has decided one.]
Your rights
- Ask what information we hold about you
- Ask us to correct information that is wrong
- Ask us to delete information we no longer need
- Ask us to stop contacting you
To make any of these requests, call us on (833) 311-1274 or write to us at 515 Madison Avenue, New York, NY 10022.
Your rights over your medical records are separate and broader, and are set out in our Notice of Privacy Practices.
[PLACEHOLDER: your attorney should confirm which state privacy laws apply to your patients and whether any additional rights or disclosures are required here.]
Children
This website is intended for adults. We do not knowingly collect information through this website from anyone under thirteen. If you believe a child has sent us information through the site, please contact us and we will delete it.
Security
The site is served over an encrypted connection. No website can be guaranteed completely secure, which is the reason for the request above: please do not send detailed health information through the contact form.
Changes to this notice
If we change this notice we will update the effective date at the top of this page. If the change is significant, we will say so clearly on the site.
